They are hard to walk in, especially when every other sky-high shoe are PLATFORMS. Those platforms have spoiled a lot of women feet. Manolo refuse to design a platform shoe because he said it's a trend and when that trend run its course, women will find it hard to walk in normal heels again. I must say that platforms are balanced evenly and can allow you to take on higher heights. I love them, but I can't really walk well in my old shoes without platforms.

the reason why model are so skinny is because of money. if a dress cost 5k to make in a size zero, it will cost 2-3 times more if make in a larger size. most designers have a budget and if theuy go over it come out their pocket. most of the time the clothing on runways are one of a kind designs. so im sure nobody test kanyes shoes because they are so expensive.
